Records Request Form

• This form is intended for current students, however, anyone with LWTech credentials can use it.
• For unofficial transcript requests, please visit the LWTech Transcript Requests page.
• Please allow up to two weeks for processing the Records Request Form.
• If needed, please use our Student ID Lookup Tool to find your LWTech ID number.

Other Ways to Request Records

The Records Request form below can only be used if you have your LWTech email account. If you no longer have your LWTech account, you may order your official transcripts one of two ways:

• Online via National Publishing Clearinghouse (fees apply)
• By mail via a printable Records Request form (fillable PDF)
