Medical Documentation Guidelines

To submit complete and compliant documentation follow these medical documentation guidelines:

  • All documentation must be typed on official letter head. No exceptions.
  • IEP or 504 plans can be used as documentation in your application. If additional documentation is needed, the DSS office will contact you. Please be advised that IEP and 504 plans are not transferrable. They can only be used to fulfill the documentation requirement of the application process.
  • Follow all DSS Documentation Guidelines below:

Specific Requirements

Documentation Must Be

  • Typed on official letterhead
  • Not more than 3 years old

Signed By

  • Psychiatrist OR
  • Medical doctor OR
  • Psychologist who specializes in the diagnosis of ADHD

Must Include

  • Clear statement of diagnosis AND
  • Summary of history and diagnosis AND
  • Current functional limitations AND
  • Recommendations for accommodations that would assist the student in a higher education setting.

Documentation Must Be

  • Typed on official letterhead
  • Not more than 6 months old

Signed By

  • Certifying professional who specializes in the diagnosis of the specific disability

Must Include

  • Clear statement of diagnosis AND
  • Summary of history and diagnosis AND
  • Current functional limitations AND
  • Recommendations for accommodations that would assist the student in a higher education setting.

Documentation Must Be

  • Typed on official letterhead

Signed By

  • Qualified professional

Must Include

  • Clear statement of diagnosis AND
  • Summary of history and diagnosis AND
  • Current functional limitations AND
  • Recommendations for accommodations that would assist the student in a higher education setting.

Documentation Must Be

  • Not older than 5 years unless conducted after the student's seventh birthday
